1. Introduction and Acceptance

Welcome to Centerline Therapy, LLC ("Centerline Therapy," "we," "our," or "us"). These Terms and Conditions ("Terms") govern your access to and use of our website located at www.centerline-therapy.com (the "Site"). By accessing or using this Site, you confirm that you have read, understood, and agree to be bound by these Terms.

If you do not agree with any part of these Terms, please discontinue use of this Site immediately.

These Terms apply only to use of this website. They do not constitute, replace, or supersede any therapy services agreement, informed consent document, HIPAA Notice of Privacy Practices, or other clinical documentation that may govern a formal therapeutic relationship with Centerline Therapy.

2. About Centerline Therapy

Centerline Therapy, LLC is a licensed mental health practice operating in the Commonwealth of Pennsylvania. Centerline Therapy provides psychotherapy services to adults in East Falls, Philadelphia, PA (in-person, Mondays) and throughout Pennsylvania (via telehealth, Tuesday through Thursday).

3. Purpose of This Website

This website serves as an informational resource about Centerline Therapy and its services. The Site allows visitors to:

• Learn about Centerline Therapy's approach, services, and clinician
• Submit a general inquiry through a contact form
• Access a link to schedule a free 15-minute consultation through our scheduling platform

This website does not collect, store, or transmit any Protected Health Information (PHI). It is not connected to our Electronic Health Records system or any HIPAA-regulated clinical platform. Clinical services, records, and HIPAA-protected communications are handled entirely through separate, HIPAA-compliant systems provided to established clients.

4. Not a Medical or Emergency Service

Nothing on this Site constitutes professional mental health advice, diagnosis, or treatment. The information provided is for general educational and informational purposes only.

5. No Therapist–Client Relationship

Accessing this Site, reading its content, submitting the contact form, or clicking the scheduling link does not create a therapist–client relationship between you and Centerline Therapy or Jacob Howell.

A formal therapeutic relationship is only established after:

• Completion of a clinical consultation
• Review and signing of all required intake and informed consent documents
• Explicit written confirmation that services have begun

Until a formal therapeutic relationship is established, no duties of confidentiality, continuity of care, or professional obligation apply.

6. Contact Form and Website Communications

This Site includes a basic contact form for general inquiries. This form is intended for non-clinical, general inquiry purposes only — such as asking about availability, fees, or whether Centerline Therapy may be a good fit.

Please do not include sensitive personal information, mental health history, diagnosis or treatment details, medications, insurance information, or any other Protected Health Information in the contact form or any email communication initiated through this website. This form is not a HIPAA-secure communication channel and is not intended for clinical communication.

Submitting the contact form does not establish a therapeutic relationship, guarantee a response within a specific timeframe, or obligate Centerline Therapy to provide services.

7. Third-Party Scheduling Platform

This Site contains a link to our scheduling platform, Sessions Health, where prospective clients may request a free 15-minute consultation. Sessions Health is a separate, third-party platform with its own terms of service and privacy policy.

By clicking the scheduling link and using Sessions Health, you are subject to Sessions Health's terms and privacy practices. Centerline Therapy is not responsible for the content, security, or privacy practices of Sessions Health or any other third-party platform.

8. Scope of Services and Geographic Limitations

Mental health services offered by Centerline Therapy are subject to Pennsylvania state licensing laws. Services may only be provided to individuals physically located in Pennsylvania or in any other jurisdiction where Jacob Howell holds an active license or is otherwise legally authorized to practice.

The availability of information about our services on this Site does not constitute an offer to provide services in any jurisdiction where such services are not legally permitted.

9. Fees and Good Faith Estimates

Centerline Therapy is a private-pay practice and does not bill insurance directly. As required by the federal No Surprises Act (45 CFR § 149.610), you have the right to receive a Good Faith Estimate (GFE) of expected costs before services begin.

For questions about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises. Specific fee information is discussed during the consultation process and outlined in your clinical services agreement.

10. Acceptable Use

You agree to use this Site only for lawful purposes. You may not:

• Use the Site in violation of any applicable law or regulation
• Attempt to gain unauthorized access to any portion of the Site or its systems
• Interfere with or disrupt the security or functionality of the Site
• Introduce malicious code, viruses, or other harmful materials
• Collect or scrape data from the Site without prior written permission
• Submit false, misleading, or fraudulent information through the contact form
